The certified community health worker role will collaborate with healthcare providers in assisting patients with education and support to improve healthcare outcomes. They provide intensive case management to patients and assist with access to care, barriers in obtaining necessary resources; both community and insurance based. In addition, they will act as a guide in navigating transitions of care during a patient’s health care journey.

 

Position Summary/Essential Duties:

1.       Assists with the recruitment and retention of research subjects

2.       Assists with study visits and procedures under the direct supervision of the PI and senior study staff

3.       Assists with scheduling subject visits and procedures.

4.       Assists with entering data into study databases (REDCap)

5.       Assists with the organization and maintenance of research documents

6.       Performs assigned activities related to clinical research studies including but not limited to: Screening participants for eligibility and recruitment, obtains informed consent, educates participants regarding study requirements and completes source documents and case reporting forms.

7.       Performs other duties as delegated and trained by the study team.

Experience and Knowledge:

•    Follows strict procedures and regulations for all sample processing operations (fingerstick blood collection)

•    Familiarity with all lab screening equipment needed to perform sample processing.

•    Maintains complete and accurate records of all screening procedures (BP, HR, height, weight for BMI, fingerstick) and other study procedures

•    Notifies supervisor and study team of any errors in sample processing procedure in a timely manner and documents per protocol requirements.

•    Consistently demonstrates behaviors which positively impact the progress of research

•    Daily contact with PI, study team, research ancillary services, and external study staff as applicable.

•    Participates in study team meetings if and when appropriate

•    Familiarity with protocols, related study documents, and research regulations and processes.

 

Education/Qualifications:

High School Diploma / GED required.

Community Health Worker Certification required.

Certification in Human Subjects Protection/CITI Training within 30 days of hire (specifically research related)

 

Position Details/Preferred Qualifications:

The Certified CHW must have their certification active with the Ohio Board of Nursing at the time of interview to be considered. Previous experience in community and/or population health is preferred. Bilingual (Spanish) is also preferred. This program specializes in the Cardio Metabolic population. The certified community health worker (CHW) will collaborate with healthcare providers in assisting patients with education and support to improve healthcare outcomes, such as high blood pressure, obesity and diabetes. They provide intensive case management to patients and assist with access to care, barriers in obtaining necessary resources; both community and insurance based. In addition, they will act as a guide in navigating transitions of care during a patient’s health care journey. The position is for Monday-Friday with rotating weekends. There is also travel involved due to the nature of the role. This position will start by March 1st. It is a 5 year Grant.
